Job Description Job Description Job SummaryThe Community Living Supports (CLS) Associate provides individualized support to individuals with developmental disabilities, mental health conditions, or other challenges to enhance their independence and quality of life. This role involves assisting clients with daily living activities, community engagement, and skill development in alignment with their individualized care plans.Key ResponsibilitiesIndividualized Support : Assist clients in achieving personal goals outlined in their care plans.Provide support with daily living activities, including hygiene, meal preparation, and housekeeping.Monitor and promote safety and well-being in clients' homes and community settings.Community Engagement : Encourage and facilitate participation in community activities and events.Accompany clients on outings to build social and life skills.Skill Development : Support clients in developing skills such as money management, communication, and problem-solving.Provide coaching and encouragement to foster independence.Behavioral Support : Implement behavioral strategies as outlined in the care plan or by clinical staff.Manage challenging behaviors with professionalism and care, using de-escalation techniques as needed.Documentation and Communication : Maintain accurate documentation of services provided, progress, and incidents.Communicate effectively with the care team, families, and supervisors about clients’ needs and changes in behavior or health.Collaboration : Work as part of a multidisciplinary team to provide comprehensive care.Attend team meetings and training sessions as required.Compliance : Adhere to all state and federal regulations, including HIPAA and other confidentiality guidelines.Follow agency policies, procedures, and safety protocols.QualificationsEducation and Experience : High school diploma or equivalent required; associate's degree or higher in social work, psychology, or a related field preferred.Previous experience working with individuals with developmental disabilities or mental health conditions is an asset.Skills and Competencies : Strong interpersonal and communication skills.Patience, empathy, and a commitment to person-centered care.Ability to handle stressful situations with professionalism.Basic computer skills for documentation and reporting.Requirements : Valid driver’s license with a clean driving record (if applicable).Ability to pass a background check and drug screening.Certification in CPR and First Aid (or willingness to obtain upon hire).Blood borne pathogens CertificationRecipient Rights trainingPhysical DemandsAbility to perform physical tasks such as lifting (up to 50 lbs), bending, and assisting with mobility.Willingness to work flexible hours, including evenings, weekends, and holidays as needed.
