Requirements
Candidates must be 18 years old and possess and maintain a current and unrestricted Illinois CNA Certification. Must have a valid Illinois Driver’s License with good driving record. Experience working in long-term care, behavioral health and/or geriatric health is preferred.
Job Summary
Qualified candidates require the ability to:
§ Arrange offsite medical appointments in accordance with physician’s orders.
§ Verify that resident benefit funding will cover offsite services.
§ Create appointment schedule and verify necessary paperwork is prepared in advance.
§ Drive residents to and from doctor appointments and stay with residents on appointments as needed.
§ Pick up new admissions from place of residence, facility, or hospital (some traveling will be involved).
§ Assemble, check contents, and maintain medical records for both active and discharged residents.
§ Compile and submit information from records as required for insurance companies, funding entities, correctional/judicial system and any other authorized individuals or entities.
§ Process referrals for current resident wishes for alternate placement in conjunction with Social Service department.
§ Conduct regular audits of active resident records to ensure all required documentation is present and timely.
§ Establish and oversee a procedure to ensure resident records do not leave the Medical Records area except as authorized in facility policies and procedures.
§ Understand and effectively carry out oral and written instructions.
§ Make observations and record properly in appropriate records.
§ Maintain confidentiality.
§ Be courteous and professional at all times.
§ Always ensure and promote resident dignity.
§ Perform routine care tasks and give simple nursing care to patients when not transporting patients.
§ Other duties as assigned.
