GENERAL SUMMARY: The Oncology Social Worker provides counseling and other supportive services to patients and families receiving cancer care at Sibley, primarily in the out-patient setting. Also acts as a consultant to physicians and Sibley staff regarding psychosocial issues, community resources and financial needs.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Provides counseling to individuals and their families throughout the cancer experience, primarily in the out-patient setting based on self-referral or physician/staff referral.
2. Participates with the Cancer Committee and designated physicians in the identification of psychosocial needs of cancer patients/families being treated at Sibley or in affiliated physician offices in the community.
3. Facilitates a general cancer support group, usually twice a month, and plans and participates in other Cancer Center sponsored functions that meet the psychosocial needs of patients and families.
4. Identifies out-patients with complex discharge planning needs. Recommends options for post treatment care based on required services and available resources. Works collaboratively with physician, patient, other patient care providers, and family members as appropriate to develop discharge plan. Facilitates multidisciplinary Patient Care Outcome meetings as needed to discuss selected complex and challenging cases, and to develop specialized multidisciplinary plans for post treatment care.
5. Acts as a consultant to Case Coordinator for in-patients with complex discharge planning needs and makes recommendations for post hospital care based on required services and available resources.
6. Documents activities in the medical record.
7. Assesses need for grant-funded transportation services and coordinates transportation logistics with patient/family, provider and Cancer Center.
8. Serves as liaison to external providers and payors to arrange for appropriate post treatment services.
9. Facilitates transfer of information among healthcare professionals to promote smooth patient transition between services and levels of care.
10. Identifies patients with special age specific needs for protective services such as long term care ombudsman, court appointed conservator, or adult protective services, and makes referral.
11. Identifies patients with special age specific needs for post treatment care related to decreased independence in the elderly. Recommends appropriate community resources.
12. Identifies patients with special financial needs and acts as a facilitator to obtain pro bono care, insurance coverage, Social Security Disability or other services based on eligibility.
13. Identifies cases with questions regarding medical necessity and appropriateness of hospital services, and makes referral to Physician Advisor member of Clinical Resource Management Committee.
OTHER DUTIES AND RESPONSIBILITIES:
1. Identifies staff learning needs related to oncology social work services and conducts formal and informal educational sessions in conjunction with unit manager and staff needs.
2. Observes infection control and safety practices.
3. Performs other duties as required.
4. Professional Responsibilities:
a. Demonstrates commitment to professional growth through attendance at educational programs within the hospital as well as those offered by accredited organizations.
b. Maintains active membership on one professional and/or specialty organization.
c. Participates on appropriate hospital committees and teams.
d. Actively seeks new methods to improve service in Case Coordination by identifying best practices utilized by other institutions and reviewing current research.
MINIMUM REQUIREMENTS:
Skills and Abilities Required:
a. Ability to communicate effectively with patient care providers and other health care team members.
b. Maintains knowledge of local community resources.
c. Knowledge of age specific characteristics as described in Sibley Memorial Hospital Age Specific Quick Reference.
d. Maintains patient confidentiality.
e. Ability to work flexible hours as is required.
f. Ability to access hospital information system programs.
g. Ability to move about the hospital as required.
Minimum Education and Experience:
a. Licensed to practice as a Social Worker in the District of Columbia.
b. Masters degree in Social Work from an accredited school.
c. Minimum of 2-5 years social work experience in a health care setting.
d. Current CPR Certification required.
SUPERVISION RECEIVED FROM: Director of Oncology Services
Salary Range: Minimum $35.41 per hour - Maximum $58.42 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
