The Position: Care Management Nurse RN or LPN
Employment Status: Full Time
Hours per two-week pay-period: 72
Essential Job Functions:
- Develop, implement and monitor personalized care plans for clients ensuring all services are aligned with their health and wellness goals
- Serve as the primary point of contact for clients and their families, providing regular updates on care plans and addressing concerns.
- Collaborate with healthcare providers, social workers, etc., to ensure clients receive comprehensive and coordinated care.
Minimum Qualifications:
- Graduated from an accredited school of nursing
- Valid Montana Nursing License RN or LPN or current temporary license
- Previous experience in caring for patients with chronic disease.
Certifications and Licenses
- Valid Montana Driver's License
- CPR
Position Summary:
Sidney Health Center is seeking a Care Management RN or LPN to join our I CARE team that provides a flexible schedule and remote possibilities. The Care Management Nurse will work in collaboration with primary care clinicians and healthcare teams. They act as service liaison, ensuring patients receive comprehensive and coordinated care and maintain accurate and up-to-date timely records of patient interaction, care plans and changes in health status in the EHF, coordinate and manage high-risk, chronically ill patients through Chronic Care Management, utilizing health coaching techniques to assist patients with self-management of their chronic disease and lifestyle changes to mitigate health risk. Essential duties of the position include excellent people skills, organizational skills, and the ability to work well with all staff.
