 The schedule for this position is Tuesday through Saturday, 8:00am - 4:30pm OR 8:30am - 5:00pm. This position requires availability to be on call.


JOB PURPOSE:   

This position is responsible for providing direct case management services to tenant of the Siena Francis House Permanent Supportive Housing Programs (PSH). The Case Manager will provide screenings for program admission, initial and annual assessments, service planning, advocacy, and long-term case management with a goal of achieving and maintaining housing stability for each tenant. This position will also provide case management services to include, but not limited to the following case management functions: assessment of needs and community referrals and linkage based on the tenants needs, education and information to increase tenant skill in maintaining housing, goal setting, problem solving and linkage to wrap around services as needed. The PSH Case Manager will provide trauma-informed, housing-first, and harm reduction services to tenants in PSH. 

 

Siena Francis House has 90 PSH units in the Rooftop Garden Apts. and the Cottages; they are site-based, permanent supportive housing programs and this case management position will be responsible for the management of 35-40 tenants and potential tenants at a given time. Siena Francis House uses a critical time intervention model for the approach to housing stability.  

 

ESSENTIAL JOB DUTIES

  • Conducts screenings to determine program eligibility requirements are met for referrals, comprehensive assessment to develop service plans and prioritize goals, on-going case management to include assessment of goal attainment and barriers to goal achievement, and discharge and after-care planning when appropriate. 
  • Works with tenants to develop and assess progress on service plans according to individual goals and objectives.
  • Ensure that tenant interactions are person-centered, goal-focused and reflect each tenant service plan.
  • Maintain contact with all tenants on a scheduled basis, based on evidence-based approaches such a critical time intervention and tenant needs.
  • Provides comprehensive case management services to tenants to include medical, physical, emotional, psychosocial, financial assistance and housing needs.
  • Implements processes to serve housing tenants consistent with best-practices and outcome measures for PSH.
  • Assists in the collection of data and other information for reporting purposes.
  • Maintains appropriate records and document all tenant interaction and maintain accurate paperwork/reports. 
  • Follow MACCH workflows and training guidelines and ensure compliance with data standards and record-keeping as required by internal and external stakeholders.
  • Perform annual re-certifications and inspections for each tenant and accurately update files, as necessary.
  • Adhere to all tenant confidentiality requirements & standards.
  • Communicate effectively in oral and written forms with tenants, their families, and their significant others, coworkers, supervisors, other service agencies and the community.
  • Coordinate with local agencies to provide services that meet the needs of PSH tenants.
  • Work collaboratively with building management and Supportive Services team to increase stability and retention of tenants in PSH housing. 
  • Initiate and encourage tenant involvement in community-building activities, such as community meals, holiday celebrations and support groups.
  • Report programmatic issues to the Case Management Supervisor based on urgency of the issue.
  • Represents Siena Francis House in MACCH Coordinated Entry workflows and activities. Attends and participates in as needed in MACCH HRT meetings and other PSH related meetings, trainings, and activities as appropriate.
  • Ability to maintain regular and punctual attendance is a requirement of this position as it requires a high-level of interaction with tenants and team coordination. 
  • Participate in applicable meetings, trainings, conferences, etc. as required both internally and externally.
  • Other duties as assigned.
  • This position will rotate on-call responsibilities with other Siena Francis House Case Managers.

 

JOB REQUIREMENTS

  • Strong case management skills. 
  • Knowledge of issues surrounding homelessness, including mental illness, domestic violence, chemical addictions, criminal behavior, and dysfunctional thinking.
  • Must have the ability to react quickly and calmly in an emergency and recognize symptoms of common mental and physical illness and make appropriate referrals for treatment.
  • Possess knowledge of local community agencies, programs, and available tenant resources. 
  • Ability to work with diverse populations and all levels of people within the workplace. 
  • Strong verbal, written, and interpersonal skills.
  • Ability to work independently in a fast-paced environment and be organized with extreme attention to detail. 
  • Continued development of resiliency skills and practice.
  • Proven ability to work independently in a fast-paced environment and be organized and detail oriented.

                

EDUCATION AND EXPERIENCE REQUIREMENTS

 

  • A bachelor’s degree in social work, sociology, psychology, or related field is preferred; considerations will be given to candidates who have an associate degree and/or three years employment experience in social services and case management.
  • Good writing skills. 
  • Ability to develop and implement programs.
  • Ability to prepare narrative and statistical reports.
  • Strong social services background. 
  • Commitment to the promotion of social and economic justice.
  • Ability to incorporate diversity and culturally sensitivity into programs.
  • Excellent communication and interpersonal skills.
  • Basic working knowledge of Windows (MSWord, Excel, and publisher). Good working knowledge of e-mail and Internet. 
  • Ability to relate to, and to work and communicate with at-risk adults from various socio-economic and ethnic backgrounds.

 

WORK ENVIRONMENT
The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. The work environment is an office setting with varying degrees of temperatures and noise levels. The work environment may have extended working hours, frequent time constraints, and shifting priorities, contributing to physical and mental stress.
