What are the responsibilities and job description for the Nurse Practitioner (APRN) position at Sierra Nevada Specialty Care?
Reports To: Practice Manager
Dialysis Rounding:
Sees assigned patients during dialysis for 2-3 limited focus encounters per patient per
month and PRN comprehensive encounters
Collaborates with Clinic Director, Medical Director, and Supervising Physician regularly for
feedback regarding individual cases, treatment plans, and practice philosophy and goals
Works closely with dialysis unit staff to ensure appropriate and adequate communication
regarding plan of care
May be required to be present at multiple locations within a single work day
Clinic/CKD Clinic:
Responsible for direct patient encounters in the clinic setting for ESRD, CKD follow up, CKD
education, Modality education, and hospital follow up
Collaborates with Physicians in clinic and Supervising Physician regularly for feedback
regarding individual cases, treatment plans, and overall practice philosophy and goals
Provides clinical support as necessary including all aspects of direct patient care
Remains up to date with current recommendations regarding CKD disease management
from RPA, ASN, NKF, and other relevant professional organizations
Regularly reviews CKD clinic protocols and discusses recommendations for revisions and/or
improvements based on national updates, best demonstrated practice and hands on
experience
Actively participates in CKD patient educational programs including development, revision,
and improvement of educational content and delivery methods
General Responsibilities:
Plays an active role in promoting quality patient care and overall practice success
Work closely with Physicians, Ancillary Staff Members, and Management to improve patient
care and outcomes
Establish and maintain professional community relationships with outside clinics/hospitals
and physicians/physician extenders and physician offices as necessary
Work with management to set and achieve goals for productivity and performance
Provide leadership/mentor support for clinical staff as needed
Demonstrate professionalism at all times, promoting a friendly work environment and
providing excellent customer service
Minimum Requirements:
Master’s Degree from an accredited university, health sciences preferred and current state licensure to
practice as a mid-level provider (Advanced Practitioner of Nursing or Physician’s Assistant) in Hawaii is
required. Must have the ability to work in a fast paced environment; excellent communication and
computer skills; 3 years of experience in medical pharmaceutical industry.

Pay: $126,001.00 - $180,574.00 per year
