What are the responsibilities and job description for the Clinical Staff Pharmacist position at Sinai Chicago?

1. Medication Order Management o Reviews medication orders for completeness and appropriateness, including indication, patient-specific attributes that warrant individualization of therapy (or avoidance of therapy entirely) drug / food interactions, therapeutic duplications and allergies o Applies pharmacotherapy to evaluate appropriateness of medication, dose and schedule– including therapy individualization based on age, end-organ function and co-morbidities o Addresses clinical concerns collegially about medications orders, pertinent labs and/or disease states with prescribers and allied health care professionals o Identifies problems in the medication use process for the organization and works to facilitate improvements in the process. o Reviews medical record for patient response, occurrence of adverse events, and omission of therapies o Executes departmental, clinical and operational protocols. Effectively utilizes guidelines and procedures to optimally impact patient care o Documents medication interventions and occurrence reports o Conducts medication histories and medication reconciliation o Promotes and demonstrates rational, cost-effective drug therapy o Is familiar with the hospital formulary and appropriately auto-substitutes to the formulary equivalent. o Documents patient care activities appropriately through use of Clinical Intervention System. Enters pending clinical interventions for all patients, as appropriate per medication, including but not limited to warfarin, aminoglycosides, vancomycin, and non-formulary entries. o TPN screening: ï‚§ Contacts physician on any unusual order ï‚§ Familiar with normal concentrations of additives and evaluates with regard to patients serum levels ï‚§ Demonstrates calculations of fluid, electrolyte and calorie needs. 2. Provision of Decentralized Pharmaceutical Care o Units: general medicine, surgery, cardiology, rehabilitation-medicine, psychology, oncology o Serves as a medication expert to unit staff. Intervenes before or at the time of ordering of medications o Attends patient care rounds, when appropriate 3. Ability to accurately enter medication orders (including chemotherapy) in a timely manner. Accurately check AMDS (Automatic Medication Dispensing System), IV admixtures, patient specific medications, prepacked medications. Responsible for the proper handling, storage and security of medications in the pharmacy and medication rooms. Ensures that all orders are filled accurately. Verifies the accuracy of all orders filled by Pharmacy Technicians. Prioritizes work flow and fills orders by level of urgency. Provides follow-through on arrival orders. 4. Transitions of care. Provide the transitions of care support for the patient, from medication history & reconciliation through the post-discharge period and patient education. Provides patient education, counseling, and HIPPI introductions. Provides patient specific rounds when required. 5. Works collaboratively with health-care providers to implement and maintain innovative disease management programs and clinical pharmacy services. 6. Contributes to pharmacy quality and medication safety initiatives: reporting of ADRs, medication errors and other issues/concerns. Identifies and engages in organizational and department quality improvement activities to improve patient care, medication use process, and pharmacy operations. 7. Ensures pharmacy technicians complete daily logs for the refrigerator, hood cleaning, chemotherapy, etc. Maintains IV batch, mini-bag plus, cartfill logs. Maintains accurate controlled substances records and signs all appropriate documentation (including Narcotic Delivery Sheets). Performs all required IV, TPN and chemotherapy safety checks. Completes all required chemotherapy documentation. Takes required protective measures. Understands compliance with USP 795, 797 and 800 standards. 8. Oversees workflow, complies with protocols/policies & procedures/guidelines and works collaboratively with both pharmacy and hospital staff to optimally meet the medication needs of the patient 9. Serves as a resource for physicians, nurses, hospital staff, and patients regarding medication therapy 10. Exemplifies organizational mission and values of respect, integrity, quality and teamwork as consistently evident in behavior 11. Educates and trains residents, students, interns, colleagues and other health care professionals as well as supervises technicians, residents, interns, or trainees in their job tasks 12. Responsible for the medication use process of preparing and dispensing medications following medication use policy and all laws, regulations, and standards applicable to pharmacy practice. Follow medication safety standards and enforces The Joint Commission/HFAP Medication Management standards. Notifies supervisor of any hazards or non-compliance immediately. Participates in safety/disaster drills and in-services. 13. Uses pharmacy automation to assist in the dispensation, verification and distribution of medications 14. Keeps Pharmacy Manager & Pharmacy Buyer informed of abnormal use of items. Follows procedure for high cost items and utilizes shift report tool and shortage board to communicate. 15. Provides operational support in the Central Pharmacy or in a Decentralized capacity, as determined by departmental need 16. Is responsive to concerns or requests of nurses, patients, doctors, employees and the public. Cooperates with others within and outside of the department. Coordinates work efforts. Offers assistance to others when needed or in special situations. Communicates information clearly and accurately in a courteous manner and tone. Follows up as appropriate to ensure messages or instructions are understood. Makes every effort to provide for a positive patient experience during encounters. Utilizes the Acknowledge, Introduce, Duration, Explanation, and Thank You (AIDET) methodology when talking to customers/patients. 17. Drug Information: Keeps current on new drugs, answers questions timely and accurately. 18. Covers assigned shifts as determined by departmental need. 19. Performs other related duties as required. 

MINIMUM EDUCATION:

• Graduate of a college of Pharmacy accredited by the American Council on Pharmaceutical Education (ACPE). Doctor of Pharmacy and hospital pharmacy residency preferred. 

WORK EXPERIENCE:

• Completed a PGY1 Pharmacy residency or one year of hospital pharmacy experience preferred. KNOWLEDGE & SKILLS: • Excellent drug knowledge base including pharmacokinetics, dosing and nutritional support. • Good computer skills. • Effective verbal and written communication skills.

REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS:

• Licensed as a Registered Pharmacist in the State of Illinois. • Completion of a Pharmacy Practice Residency (PGY1) or 1 year of hospital pharmacy experience or BCPS certification preferred within 24 months of hire • ACLS certification

This position’s responsibilities would be split between the performance of duties below as an inpatient staff pharmacist, and would be involved with the operations of our outpatient infusion center including, but not limited to, review/generation of orders for outpatient chemotherapy, inventory maintenance, patient counseling.
