We are seeking a dedicated Care Coordinator to join our team. The Care Coordinator will play a crucial role in providing support and coordination for individuals in need of care services.
Responsibilities
- Coordinate care services for clients based on their individual needs
- Conduct assessments to determine clients' requirements and develop care plans
- Collaborate with healthcare providers, social workers, and other professionals to ensure comprehensive care
- Monitor and evaluate the effectiveness of care plans and make adjustments as necessary
- Provide guidance and support to clients and their families throughout the care process
- Maintain accurate and up-to-date records of client interactions and progress
Requirements
- Possession of a certificate in Social Work or related field is preferred
- Strong conflict management skills to handle challenging situations effectively
- Experience in law enforcement, supervising, or militarise is a plus
- Familiarity with motivational interviewing techniques
- Proficiency in case management to ensure efficient coordination of care
