Responsibilities
- Plan, develop, and implement social service programs to meet the emotional and social needs of residents.
- Serve as a liaison between residents, families, and facility staff to ensure effective communication and care coordination.
- Assist residents and families in understanding and adjusting to illness, disability, or placement in a long-term care facility.
- Provide support with care planning, discharge planning, and transitions of care.
- Maintain compliance with state, federal, and facility regulations.
- Document social service assessments, progress notes, and care plans accurately and timely.
- Supervise and mentor social services staff, promoting a positive and supportive team environment.
Requirements
- Bachelor’s degree in Social Work (BSW) required; Master’s degree preferred.
- Current state license (LSW or equivalent) in good standing.
- 2 years of experience in a Skilled Nursing Facility (SNF) strongly preferred.
- Knowledge of federal and state regulations related to long-term care.
