What are the responsibilities and job description for the MDS-LVN position at Skyline Healthcare Center-San Jose?
SUMMARY
Provides assistance in Resident care planning activities, including the MDS and MMQ processes. Participates in timely completion of MDS process (Minimum Data Set, discharge and re-entry tracking forms).
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Assists the Resident Assessment Coordinator in fulfilling responsibilities, as directed.
- Completes assessments of Residents that directs the plan of care.
- Conducts orientation sessions on the MDS process to all employees involved.
- May coordinate information systems operations and education for the clinical department.
- Responsible for contacting Medicare benefits hotline as required per company guidelines.
- Develops schedule of due dates for periodic assessments.
- In the absence of the Resident Assessment Coordinator, may perform duties of that position with oversight by a Registered Nurse (if applicable).
- Completes required forms and documents in accordance with company policy and state and/or federal regulations.
ENTRY QUALIFICATIONS
- Graduate of an accredited school of Nursing.
- Valid State registration as an LPN/LVN
- Knowledgeable of OBRA, Perspective Payment System, and MDS process.
