Position Overview
We are seeking a board-certified Nurse Practitioner (NP-C or FNP-C) to join our team of dedicated providers. The ideal candidate will have a strong clinical background in family medicine or internal medicine and a genuine interest in providing holistic, patient-centered care. Key Responsibilities
- Conduct comprehensive patient evaluations including history, physical examination, and risk assessment.
- Diagnose and manage acute and chronic conditions such as diabetes, hypertension, asthma, infections, and mental health concerns.
- Develop and implement individualized care plans based on clinical findings and patient goals.
- Order and interpret diagnostic studies including lab work and imaging.
- Prescribe medications and monitor treatment effectiveness, adjusting plans as needed.
- Provide preventive care, including screenings, immunizations, and patient counseling.
- Educate patients and families on disease prevention, healthy lifestyle, and chronic disease management.
- Coordinate care with other clinic team members, specialists, and external providers.
- Maintain thorough and timely documentation in the electronic medical record (EMR).
- Participate in quality improvement projects and clinical meetings.
- Participation in appropriate documentation and billing efforts.
- Participation in process improvement exercises.
Qualifications
Required:
- Master’s or Doctorate degree in Nursing from an accredited program.
- Current Illinois APRN license and certification as a Family Nurse Practitioner (FNP-C or equivalent).
- BLS Certification (ACLS preferred).
Preferred:
- Experience with EMR systems (e.g., Epic, Meditech, eClinicalWorks).
- Familiarity with value-based care models and care coordination strategies.
- Experience with diverse and underserved populations.
- DEA license and controlled substance registration in Illinois.
- Minimum 1–2 years of experience in primary care or a similar outpatient setting.
