Smithtown Center for Rehabilitation and Nursing Care -

We are looking for an experienced and compassionate Social Worker to join our team. The ideal candidate will have a passion for helping others and a commitment to providing quality services to those in need. The successful candidate will be responsible for providing counseling services, developing treatment plans, advocating for clients, and connecting individuals with community resources. Additionally, the Social Worker will be expected to maintain accurate records and ensure compliance with all relevant regulations.

Salary : $30- $35 / hr

Responsibilities for a social worker :

  • Assists residents with emotional adjustment to facility.
  • As directed, interview new residents and records, social history and assessments.
  • Completes the social services portion of the MDS.
  • Schedules and Attends and participates in interdisciplinary care plan meetings.
  • Writes summaries and progress notes as directed.
  • Assists in conducting a pre-discharge evaluation
  • Assists with discharge planning. Follows up after discharge.
  • Writes discharge summaries as assigned.
  • Develops a relationship with each resident

    • Qualifications for a social worker :

  • Degree in Social Work
  • Bachelor's Degree
  • One to Three years of professional experience and / or training; or equivalent combination of education and experience.

    • Job Type : Per diem to start and then FT starting in March

    Salary : $30 - $35
