Responsibilities:
- Collaborate with LSW to develop and implement patient-centered care plans
- Provide emotional support to patients and families, addressing their psychosocial needs and concerns
- Conduct social work assessments, identifying patients' strengths and challenges
- Develop and implement interventions to improve patient outcomes and quality of life
- Coordinate with community resources and referrals as needed
- Document patient interactions and progress in accordance with facility policies
Requirements:
2 years of experience preferred
High school diploma or equivalent required; A.A.S preferred
Strong communication and interpersonal skills
Ability to work effectively in a fast-paced environment
Commitment to providing high-quality patient care
