Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care to our underserved population here at Saint Joseph, Missouri. This role requires a strong commitment to patient advocacy and the ability to work collaboratively with other healthcare professionals to ensure optimal patient outcomes.

ESSENTIAL JOB DUTIES

1. Performs direct patient care and telehealth services using guidelines while operating under a collaborative practice agreement.

2. Initiates a collaborative health assessment of the patient including physical, psychological, social, and environmental status as well as the client’s ability to function independently.

3. Identifies individualized expected outcomes for patient/family, as well as interventions that assist the patient in achievement of the outcomes.

4. Develops, implements, and monitors the plan of care through a multi-disciplinary and collaborative team process.

5. Prescribes and dispenses medication per collaborative agreement.

6. Evaluates the multi-disciplinary plan of care and patient’s progress toward the attainment of outcomes, modifying as necessary.

7. Acquires and maintains current knowledge in nursing practice and an in-depth knowledge base of the patients served by the Stuber Health Center.

8. Collaborates with patient, family, and other health team members to provide comprehensive patient care.

9. Role models professional behavior and assumes leadership in clinical areas of expertise.

10. Presents a courteous and helpful demeanor to all patients, visitors and to other Stuber Health Center employees and medical staff.

11. Performs other designated clinical duties within the scope of the nurse practice act of the state of Missouri.

OTHER JOB DUTIES

1. Actively participate in performance improvement activities including but not limited to medical clinic audit and other agency staff member’s annual evaluations.

2. Actively participate in Medical Team and agency activities related to client service and advocacy for Stuber Health Center’s patient population.

3. Effectively participate in other clinical and agency duties as assigned, including but not limited to client triage, community education presentations, activities related to agency goals, and the establishment and maintenance of linkages with other providers in the community.

4. Regularly seek to update and maintain a current level of knowledge relative to professional practice, and maintain continuing education requirements necessary for licensure.

5. Appropriately perform other activities not described above that are deemed appropriate by the agency.

EDUCATION/WORK EXPERIENCE

1. Current licensure to practice as an Advance Practice Nurse in the State of Missouri.

2. Well-developed clinical skills in the areas of client care enumerated in the list of essential job duties above.

3. Well-developed organizational skills necessary to assure smooth operations of the medical clinic.

4. Strong interpersonal skills necessary to work with disadvantaged and underserved populations and to collaborate with other medical team members, other staff members, and personnel of other agencies.

5. Comfortable working with low-income clients.

SPECIFIC REQUIREMENTS

1. Possesses the ability to make independent decisions when circumstances warrant such action.

2. Possesses the ability to communicate and deal tactfully with staff, visitors, clients, and the general public.

3. Able to work harmoniously with other medical and non-medical staff members.

4. Able to follow written and oral instructions.

5. Able to work independently.

6. Possesses basic knowledge in the use of computers.

7. Able to communicate and/or process knowledge, skills, or abilities to effectively interact with and care for the adult and geriatric clients at SHC.

PHYSICAL DEMANDS

1. Able to move frequently throughout the day.

2. Able to cope with interruptions, be flexible, and be a team player.

3. Possesses sight/hearing senses or uses prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met.

4. Functions independently, have flexibility, personal integrity and the ability to work effectively with other personnel.

5. In good general physical health and demonstrates emotional stability.

6. Able to relate and work well with ill, disabled, emotionally upset, and at times hostile people.

WORKING CONDITIONS

1. Sits, stands, bends, lifts and moves intermittently during working hours.

2. Subject to frequent interruptions.

3. Must be able to communicate with SHC staff, visitors, clients, etc.

4. Is subject to potentially hostile and emotionally upset staff, visitors, and clients.

5. Is involved with clients, staff, visitors, and social agency personnel under all conditions and circumstances.

6. May be exposed to infectious disease.
