Job Title: Patient Enroller – Chronic Care Management (CCM)

Location: Redding, California

Employment Type: Part-Time (2-3 days per week)

Schedule: Monday to Friday, 8am - 4pm

This is a 1099 Independent Contractor role with the potential for a full-time salaried position. FLEXIBLE schedules available including Part Time, Per Diem, and Weekends.

We are seeking a reliable, patient-focused individual to support enrollment efforts at a clinic in Redding, CA, for our Chronic Care Management program. This role involves working directly with patients and clinic staff to ensure a smooth onboarding process to the CCM platform.

Medical Assistant certification is a plus, but not mandatory.

Qualifications:

• Warm, empathetic personality with a genuine desire to help patients

• Strong verbal and written communication skills

• Comfortable coordinating with clinic staff, nurses, and physicians

• Familiarity with installing and navigating apps on Android and iPhone devices

• Willingness to learn basic troubleshooting for mobile apps and Bluetooth devices

Key Responsibilities:

• Assist patients in downloading, installing, and setting up the Evolko HealthRadar app on their smartphones

• Support patients in connecting Bluetooth health devices (e.g., blood pressure monitors) and completing their first check-in

• Respond to patient questions and guide them through common setup or app-related issues

• Follow the standardized enrollment process and checklist for registering patients in the CCM program

• Participate in daily check-in calls with the Evolko coordinator to report progress and resolve issues

• Reach out to inactive patients via the clinic’s official phone system to encourage engagement
