Job Overview
We are seeking a dedicated and compassionate Registered Nurse to join our healthcare team. The ideal candidate will have experience in providing care to children, working with individuals with developmental disabilities, and a strong background in home care settings. This role requires a commitment to delivering high-quality patient care, including hospice and palliative medicine, while adhering to established standards and protocols.
Responsibilities
- Assess and evaluate patient health needs through comprehensive physical examinations and medical histories.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Administer medications and treatments as prescribed, ensuring compliance with safety standards.
- Monitor patients' progress and adjust care plans as necessary based on their evolving needs.
- Provide education and support to patients and families regarding health management and disease prevention.
- Maintain accurate documentation of patient care activities using electronic health record systems such as Epic.
- Participate in multidisciplinary team meetings to discuss patient care strategies and outcomes.
- Conduct MDS coding for appropriate reimbursement and quality assurance purposes.
Qualifications
- Current Registered Nurse (RN) license in Connecticut (CT).
- Experience working with children or individuals with developmental disabilities is preferred.
- Background in home care or hospice palliative medicine is highly desirable.
- Familiarity with MDS coding standards is a plus.
- Strong communication skills, both verbal and written, for effective patient interaction.
- Ability to work collaboratively within a healthcare team while demonstrating leadership qualities.
- Commitment to ongoing professional development and adherence to best practices in nursing.
Job Types: Full-time, Contract
Pay: $82.00 - $85.00 per hour
