SOUTH BAY POST ACUTE CARE is looking for a dynamic and professional Nurse Case Manager to join our Amazing Team!
A Case Manager work closely with the facility Social Services department, as the primary liaison between patients, families, physicians and health plans. Minimum of two years of hands on nursing experience preferred. Although general understanding of insurances if preferred, it is not a requirement. We are willing to train the right individual.

Requirements:
- Current and unencumbered LVN OR RN license from the State of California.
- Active CPR Card
- General knowledge about Nursing Practices, Policy and Procedures, Medical Plans (preferred)
- PCC/Electronic Medical Record knowledge (preferred)

Pay: $33.00 - $46.00 per hour
