Supportive Housing Adult Care Coordinator Covington County Full Time

Position Overview:

We are seeking a dedicated and passionate Supportive Housing Case Manager to join our team. In this role, you will be expected to work with an intensive case load of twelve adults with mental health illnesses.

Qualifications:

• Bachelor’s degree in a human service related field
• Case management trained preferred.

Responsibilities:

• Schedule home visits.
• Obtain relevant information from consumer to write plans of care.
• Establish liaisons with other service agencies in the community.
• Provide transportation and assist consumers in obtaining the services they are entitled to receive.
