The Reentry Case Manager will be responsible for providing critical care and case management services for inmates housed within the SCORE facility. Reporting to the Jail Chief, this position involves conducting comprehensive health assessments that include mental health evaluations, assessing physical health needs, etc. and addressing risk factors impacting inmates' well-being. This requires providing guidance to interdisciplinary care teams with training, consultation, and oversight, and directly supervising one reentry coordinator. Additionally, the Reentry Case Manager will coordinate referrals to community providers in support of individualized care plans. This position will design and implement care initiatives while ensuring compliance with regulatory standards and organizational policies.

South Correctional Entity (SCORE) is a regionally operated jail in Des Moines, WA serving the confinement needs of 6 owner cities and several contract agencies with a total capacity of 802 inmates. Work is performed in a 164,000 square foot jail environment and involves hazardous duty on occasion. Work is subject to frequent interruptions and higher than average noise within enclosed areas.
Reentry Case Manager may be exposed to physically confrontational situations, personal danger, and bio-hazardous materials.
JOB REQUIREMENTS:
· Master's degree in social work, counseling or related field required, plus a minimum of 3 years of clinical experience in mental health, substance use, or integrated care settings.
· Strong knowledge of social determinants of health and community-based resources.
· Bilingual or multilingual proficiency is a plus. Excellent verbal and written communication skills.
Due to the security requirements of a jail, a stringent background investigation is conducted on all candidates considered for hire. This includes fingerprinting, criminal history, a polygraph exam, psychological evaluation, and a medical exam including a drug-screen.
PREFERRED QUALIFICATIONS:
· Preferred experience in supervising multidisciplinary care teams, or experience with Electronic Health Record (EHR) systems.
· Training in evidence-based practices such as Motivational Interviewing (MI), Cognitive Behavioral Therapy (CBT), or Trauma-Informed Care is preferred.
LICENSE AND OTHER REQUIREMENTS:
· Actively licensed in social work (LICSW), licensed mental health counselor (LMHC), and/or licensed marriage and family counselor (LMFT) or other closely related licensure in Washington State.
· Valid Washington State driver’s license
