What are the responsibilities and job description for the Case Manager position at Southeast Healthcare?
The position provides case management services to promote recovery and wellness in adults with severe mental illness. Our Case Managers work as an integral part of an inter-disciplinary team to provide community based supports, including resource development, promoting good health, crisis intervention and treatment planning for successful goal attainment.
