Southern Christian Services for Children and Youth, Inc. provides children and family services throughout the State of Mississippi. Licensed by the Mississippi Department of Child Protection Services and certified by the Mississippi Department of Mental Health, SCSCY is a member agency of the Mississippi Association of Child Caring Agencies and is an affiliate of the National Benevolent Association of the Christian Church as well as a member of Disciples Care Network.
The Transition Facilitator provides skills training and case management services to transitional aged youth (TAY). They provide therapeutic services identified in the services plan by the TAY to assist them in their journey to independence. Transition Facilitators utilize Transition to Independence Process(TIP) and Trust Based Relational Intervention (TBRI) to inform all aspects of services provided.
REQUIRED:
- Bachelor or Master’s degree in social work, social services, or related field
- Three years’ experience in social work
- Case Management
- Strong organizational and interpersonal skills.
- Willingness to work in a team setting and to collaborate with community partners.
RESPONSIBILITIES:
- Commit to Utilizing the Transition to Independence Process(TIP) and Trust Relational Intervention(TBRI) principles.
- Provide individual comprehensive case management services utilizing the TIP model.
- Develop youth driven independent living plans with assessment of all TIP domains with the Divisional Director.
- Conduct weekly apartment/room inspections.
- Set up and maintain case files and documentation in accordance with agency’s standard.
- Conduct admissions and discharges.
- Teach and model independent living skills and coping strategies.
- Provide transportation for TAY to school, appointments, work etc. as needed and assist in developing a transportation plan with TAY.
- Provide crisis intervention services to TAY as needed according to agency policies and procedures.
- Represent P.A.L.S. and Southern Christian Services for Children and Youth Inc. in the community in a positive light.
- Other duties as assigned.
Valid MS driver's license. Maintain personal automobile liability insurance ($50K minimum).
