As our first Family Support Coordinator, you’ll play a key role in delivering whole-child care by supporting the social and emotional needs of children and families across our network. Working closely with clinic staff, community organizations, and care teams, you’ll help ensure our social work services are accessible, family-centered, and well-integrated into our day-to-day care. You’ll help us go from “reactive and fragmented” to “proactive and standardized” —supporting activities like SDOH screening, coordinating referrals to medical and community resources, and assisting in the development of programs for at-risk populations. Your work will help reduce barriers to care and strengthen how our providers support families.

Note: this is a unique opportunity to join a rapidly growing organization on the ground floor, with an opportunity to build a unique and innovative integrated care model.

Within 3 months, you have laid the foundation:

  • Begun to identify and engage patients and families for your initial panel in 1–2 pilot clinics
  • Planned & delivered an orientation session for providers and staff on the role of social work in pediatric primary care
  • Established trusting relationships with care teams and families through direct engagement and presence in the clinic
  • Collaborated on initial policies, operational workflows, and documentation tools for internal and external referrals and direct care

Within 6 months, you have ramped your panel and built repeatable processes:

  • Fully ramped your caseload across 1-2 clinics, providing compassionate, family-centered support for patients and families with complex medical and social needs
  • Developed and maintained a directory of community and medical resources to support referral and social needs navigation
  • Begun to collect and report on key metrics (e.g., number of screenings completed, referrals made, and follow-up rates)

Within 12 months, you have measured impact and optimized:

  • Maintained a full caseload of patients, providing compassionate, family-centered support for patients and families with complex medical and social needs
  • Demonstrated improved connections to medical and community resources (e.g., 75% of referred families successfully connected)
  • Supported onboarding and mentorship of new team members to ensure consistency and quality across sites
  • Refined workflows based on data and feedback, optimizing for efficiency and family experience

WHAT LEADS TO SUCCESS

  • Expertise. You have 5 years of experience in pediatric or family social work and a master’s degree in social work (MSW) from an accredited program. You’ve designed social work programs and have demonstrated expertise in SDOH and care coordination. You’ve made a tangible impact in the communities you’ve served, and you have strong relationships with community partners in middle Tennessee.
  • Compassion and Empathy. You can demonstrate compassion and empathy in working with children and families across diverse populations.
  • Biased to action. You know how to identify what matters, prioritize ruthlessly, and move forward even in imperfect circumstances. You don’t wait for a perfect plan—you execute, learn, and improve.
  • Comfort with ambiguity. You thrive in early-stage environments where the path isn’t fully mapped. You ask the right questions, form your own opinions, and adjust with changing priorities.
  • Player/Coach. You’re just as comfortable diagnosing SDOH risk as you are designing the playbook for the next 3 years. You can roll up your sleeves today and build a team tomorrow.
  • Comfortable with travel. Some local travel is expected — especially early on — as you set up services across clinics and get to know our people. We anticipate this will taper over time.

Job Type: Full-time

Pay: From $65,000.00 per year
