Position: Case Manager
Reports To: Assistant Director Case Management Operations

Position Function: Case Managers assist people 60 years of age and older through the Older Americans Act and Choices for Care Program as well as younger, disabled individuals on the Choices for Care program. Case Managers help to obtain/maintain the services needed to live as independently as possible within the framework of available resources and community supports. By assessing needs and working together with the client to develop a plan of care to meet these requests in a manner consistent with the person’s capacity and preferences for care, Case Managers act as agents and advocates for the client.
Essential Functions Include:
Case Management:
- Assess client situation and develop an appropriate plan of care with client input and agreement
- Assist clients to gain/maintain access to available services and programs such as public benefits, health care, housing, legal services, etc.
- Coordinate and monitor all services provided to a client, including but not limited to Choices for Care, Meals on Wheels, health care and social services as well as the client’s own informal care network
- Monitor, reassess and change a client’s care plan as needed and approved by the client per the Choices for Care policies and protocols
- Arrange for and/or participate in multi-agency case consultations on behalf of clients
- Maintain complete and accurate case records and reports and submit documents by the 5th working day of the month
- Direct Senior Companions and other agency volunteers as assigned to specific clients
- Attend required meetings and trainings
- Keep informed regarding regulatory changes and legislation affecting clients
- Perform other duties as assigned by Assistant Director Case Management Operations.
Knowledge, Skills and Abilities:
- Bachelor’s degree, preferred in social work, gerontology, or a related field, with at least 1 year of experience or a supervised internship coupled with a letter of recommendation regarding the student’s competency and practice skills. Associates degree, preferred in social services, with 3 years of experience or 2 years of experience coupled with a supervised internship and letter of recommendation.
- Ability to understand the need for and maintain client confidentiality
- Knowledge of issues related to elder populations and communities while maintaining client direction and confidentiality
- Strong organizational skills, including the ability to prioritize and manage a caseload as assigned
- Ability to establish partnerships and work cooperatively with a wide variety of people and organizations, independently, and as part of a team
- Ability to learn specific DAIL and SVCOA databases
- Proficient computer skills with Microsoft Office and other software programs
- Excellent written, oral, and listening skills
- Highly organized, attention to detail, self-starter and problem solver
- Access to reliable transportation and ability to travel throughout the state
- Ability to pass multiple background checks
- Ability to complete annual HIPAA trainings as required

Pay: $37,000.00 - $39,520.00 per year
