Functions:
This role is responsible for supervising and managing life services with a person-centered approach, ensuring the effective implementation of Person-Centered Plans (PCPs) through collaboration with staff. Key duties include overseeing PCP strategies, supporting individual goals, ensuring regulatory compliance, and promoting self-advocacy. Additionally, the position involves documentation, financial oversight, staff supervision, emergency planning, and policy implementation across multiple sites. The role requires building strong relationships with families, guardians, and external stakeholders to coordinate services that enhance the quality of life, independence, and well-being of persons served.

Duties and Responsibilities:
- Directly responsible for the oversight and implementation of the plan to ensure person-center support is encompassed. (Essential and Non-modifiable)
- Must attend, or make necessary arrangements if unavailable to attend, all meetings for people supported in order to ensure the appropriate information is shared with the team. (Essential and Non-modifiable)
- Responsible for ensuring that complete, accurate, and appropriate documents and documentations are in place for all plans, including but not limited to reviewing high level documentations. (Essential and Non-modifiable)
- Monitors weekly funds requests, along with finances and budgeting of expenses as it relates to the PCP. (Essential and Non-modifiable)
- Works in conjunction with the Residential/Vocational Teams to ensure staff are trained on and are implementing all outcomes identified in the plan. (Essential and Non-modifiable)
- Works in conjunction with the Residential Team regarding staff supervision, interviewing of applicants, evaluation of employee performance, training and development, and disciplinary action as it relates to the department. (Essential and Non-modifiable)
- Ensure emergency redbooks are up to date, including but not limited to active advanced directives and diets. (Essential and Non-modifiable)
- Works in conjunction with Residential Director/ House Supervisors/ Residential Team to ensure adequate coverage in the homes. (Essential and Non-modifiable)
- Implements and maintains program policies, procedures, and conditions which comply with appropriate regulatory and accreditation agencies. (Essential and Non-modifiable)
- Serves as liaison, when appropriate, with families, guardians, pertinent agencies, businesses, and funding representatives as assigned by the department director. (Essential and Non-modifiable)

- Travels to various program locations to ensure implementation of the person-centered plan in compliance with licensing and accreditation standards, as required by rule, regulation, policies, procedures, and/or directives. (Essential and Non-modifiable)

Skills Needed:
- Working knowledge of local and community-based resources that may be used by persons supported.
- Must possess working knowledge of Windows-based computers/software systems, as well as local and cloud-based storage systems, including proficiency in Microsoft 365 applications (Word, Excel, Outlook), utilization of electronic health record systems, databases, web-based software, and other tools used for documentation and service delivery.
- Working knowledge of HIPAA standards is essential, including understanding industry requirements for confidentiality, compliance, and maintaining electronic and paper file security.
- Must be detail-oriented with excellent time management skills and the ability to effectively manage competing priorities, meet deadlines, and demonstrate problem-solving skills.
- Must possess effective communication and excellent writing skills with the ability to interact professionally with diverse teams, and maintain collaborative working relationships with agency staff, persons supported, families/guardians, external stakeholders, outside agencies, and the community.
Qualifications:
- A human services professional with a bachelor’s degree in a human service field, including but not limited to social work; special education, education, rehabilitation counseling and psychology.
- A minimum of two (2) years of supervisory experience with demonstrated problem-solving techniques and the ability to effectively direct, monitor, and oversee the program.
- Must have a current, valid Illinois driver’s license and proof of automobile insurance.
- Must have reliable transportation in order to manage multiple sites. Must meet the requirements of the agency insurance carrier and drive agency vehicles as needed.
- Ability to work flexible schedules as needed. This will include working in an on-call capacity as required by the agency.
- Ability to physically handle the special needs of the individuals served by the program and to assist program staff in emergencies including the ability to lift, push, pull, stoop, twist, turn, squat, climb stairs, walk, stand stationary, transfer supported persons, and assist in lifting a minimum body weight of 150 pounds.
- Must attend and satisfactorily complete all training requirements.
Working Conditions:
- Work location includes Sparc’s Vocational Work Center, The Baxter Center, Sparc residential locations, the Sparc Administration Building, and places of employment for individuals supported. Some locations may expose the employee to mild to moderately high noise levels.
- Safety and security procedures for each work site must be followed.
- Staff in this role may be exposed to various illnesses and diseases as well as hazardous and toxic chemicals, with some exposure to physically threatening clients, bodily fluids, bloodborne and airborne diseases, viruses, communicable diseases, etc.
