Job Description
Sparx is seeking a licensed and experienced Licensed Clinical Social Worker (LCSW) to join our team on a part-time basis. The ideal candidate will have expertise in the New York State (NYS) Health Home Program, specifically focusing on children's care management. This role is critical in assisting with regulatory processes, licensure requirements, and providing ongoing clinical support.
Key Responsibilities
- Serve as the primary contact between Sparx and the lead Health Home agency.
- Assist with navigating and meeting regulatory and licensure requirements for the Health Home program.
- Provide expert clinical oversight and support for children's care management services.
- Collaborate with internal and external stakeholders to ensure compliance with NYS Health Home Program standards.
- Guide staff training and development regarding Health Home policies and procedures.
- Monitor program quality and help implement best practices to improve care coordination outcomes.
Location: New York State (Hybrid/Remote options available)
Qualifications
- Active Licensed Clinical Social Worker (LCSW) in New York State.
- Demonstrated experience with the NYS Health Home Program, particularly in children's care management.
- Strong understanding of regulatory and licensure processes within the NYS healthcare system.
- Excellent interpersonal, organizational, and communication skills.
- Ability to work independently while effectively collaborating with a multidisciplinary team.
- Passion for improving outcomes for children and families through care coordination and clinical oversight.
Preferred Qualifications
- Familiarity with case management software and electronic health records (EHRs).
- Prior experience serving as a liaison between agencies and lead health homes.
- Proven track record of managing regulatory and licensure requirements successfully.
