JOB TITLE: Substance Use Disorder (SUD) Triage Caseworker


JOB SUMMARY: The SUD Triage Case Worker will provide engagement and care services
including crisis intervention if necessary to Spectra Health patients and clients of the Any
Positive Change (APC) Project Syringe Service Program (SSP). They will engage with patients
and assess their interest/need in substance use treatment services. They will work with the
integrated team to plan and implement care plans to meet the social and emotional needs of
patients and their families at Spectra Health. They will collaborate with team members to
coordinate patient care within Spectra Health and with community agencies. The duties of the
SUD Triage Case Worker include being available for warm hand-offs from SSP program staff,
engaging with patients, assessing patients' interest and need for treatment services, connecting
patients with clinical and treatment services, assessing Social Drivers of Health (SDOH) deficits
for patients and implementing care plans to address the SDOH's components of patient's care.
The SUD Triage Case Worker reports to the Health Equity Director.

ESSENTIAL JOB FUNCTIONS:

  • Coordinate and provide care that is safe, timely, effective, efficient, equitable, and patient centered.
  • Triage patient appropriateness for treatment services.
  • Communicate with providers, staff and other agencies to coordinate patient care.
  • Examine patient needs in order to determine what social services are required.
  • Complete SDOH screenings with patients.
  • Assist patients with connecting to the appropriate social service agencies.
  • Assist patients with connecting to treatment/recovery services.
  • If appropriate, provide Free Through Recovery/Community Connect Care Coordination services to referred patients. Handle case assignments, draft service plans, review case progress and determine case closure.
  • Provide case management and follow-up with Spectra Health patients and SSP clients.
  • Ensure timely and effective use of Spectra Health services and referral resources.
  • Assist with (or conduct) outreach efforts on behalf of Spectra Health.
  • Assist with coordination and patient enrollment in Spectra Health's sliding fee program.
  • Ability to understand, navigate, and communicate both billing and insurance complexities to patients.
  • Assist patients in navigating and enrolling in health insurance options.
  • Complete Certified Application Counselor training annually to assist patients with Marketplace insurance coverage options.
  • Ability to navigate and apply financial requirements to patient situations.
  • Place follow-up calls to patients, as necessary.
  • Willingness to accompany patients to appointments within Spectra Health and with partner agencies.
  • Ability and willingness to work with Certified Interpreters on a regular basis.
  • Ability to coordinate Certified Interpreters to meet patient needs.
  • Establishes and maintains case files, referrals, and other related documents for the treatment of patients.
  • Ability to comply with data/grant collection requirements.
  • Other duties as deemed necessary.

JOB QUALIFICATIONS:


Required:

  • Bachelor's Degree in Social Work or related field
  • Credentialed as an LBSW (Licensed Baccalaureate Social Worker) in North Dakota
  • Valid Driver's License, Licensed and Insured Vehicle

Preferred:

  • Experience working in a clinical or hospital environment
  • Experience working with homeless and transient populations
  • Experience working with individuals with Substance Use Disorders
  • Experience working with New American/Refugee populations
  • Experience working with interpreters
  • Ability to speak multiple languages
  • Experience with grant program development, implementation, and evaluation

GRANT DISCLOSURE: This is a grant funded position that is subject to the terms and timeframe of a specific grant.

PHYSICAL REQUIREMENTS: The SUD Triage Case Worker is a position that exists in both an office, clinic, and community setting. When in the office setting the job requires the ability to sit for long periods of time at a desk or workstation. When in a clinical setting there would be long periods of time on one's feet, standing and moving throughout the clinical environment. Periodically, the position requires lifting or carrying items that would be appropriate in an office environment, not more than 20lbs.
