What are the responsibilities and job description for the Activity Lead - Adult Foster Care position at Spectrum Human Services?
What you will be doing: Scheduling and coordinating activities for individuals, both in their home and in the community. Additionally, plans specific activities to support individuals in reaching their personal growth goals. Communicates and coordinates all issues/information related to these activities. Reviews progress data and reports on outcomes toward goal areas. Supports culturally competent recovery-based practices and person centered planning as a shared decision making process.
QUALIFICATIONS
Education
- High School Diploma or GED preferred.
- CMH/DMH Training preferred.
Experience
- One year experience working with people with disabilities.
- Character of high moral standards as stated in licensing regulation.
- Three references deemed acceptable to Spectrum
