What are the responsibilities and job description for the Hospice Home Health Aide position at Spero Hospice?
$23 - $25 hourly
Responsibilities:- Reinforce accurate and up-to-date care records and work with the case manager to implement the patient care plan
- Mitigate issues with medications by helping to open bottles, organizing self-administered dosing, and arranging for refills, and help the patient with meal preparation and planning
- Help patients complete exercise, and physical therapy and ambulate the patient as indicated in the care plan
- Travel with patients to health care service providers, doctors’ offices, grocery stores, or for other errands, as needed
- Administer basic medical tasks like taking vital signs, measuring glucose and ketone levels, replacing bandages and dressings, and ensuring the proper use of medical devices
- Empathetic, compassionate, and kind personality with excellent organization and communication skills are desired
- Physical requirements include being able to lift up to 100 lbs, may be required to stoop, bend, reach, and transfer patients up to 250 lbs as needed
- Some college preferred, but a high school diploma or GED is required
- Carry a valid state driver’s license, current auto insurance, and have reliable transportation
- Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) is highly valuable- approved home health aide training programs from community colleges are accepted
- You must be authorized to work in the United States and live within commuting distance of West Orange, NJ to be considered for this position.
