JOB DESCRIPTION

TITLE:                                  ACTIVITY DIRECTOR

DEPARTMENT:                   Activities

REPORT TO:                        Admissions

WAGE:                                   Hourly

WORK HOURS:                  Variable – May be required to work unusual hours as needed.

                                                 

 

MINIMUM QUALIFICATIONS:

  • High school graduate
  • Excellent creative and communication skills.
  • Ability to organize, document, and implement detailed programs.
  • Outstanding interpersonal skills with high level of energy and enthusiasm.
  • Ability to perform work tasks within the physical demand requirements as outlined below.
  • Genuine caring for and interest in elderly and handicapped people.
  • Ability to comply with the Patient Bill of Rights and Employee Responsibilities.
  • Ability to comply with company and departmental safety policies and procedures.
  • Ability to read and write the English language.

RESPONSIBILITIES:

  • Develop, organize, and implement an individualized program of activities for social, emotional, physical, and other therapeutic needs of the residents within specified budget.
  • Residents shall have the right to choose the types of recreational, cultural, and religious activities and social events in which they wish to participate if such activities do not interfere with the rights of others.
  • Maintain detailed records of activity programs and participation of individual residents, identifying progress toward established care plan goals.
  • Participate in resident care planning by identifying the activities needs of residents in accordance with the medical assessment, working with Occupational Therapist as appropriate.
  • Conduct and maintains resident activity histories and assessments as required by state and federal standards.
  • Publish and distribute a monthly calendar of events and periodic facility newsletter.
  • Develop and maintain an active volunteer program.
  • Responsible for job specific training of departmental employees.
  • Participates in Interdisciplinary team meetings, i.e., care plans and writes progress notes.
  • Maintains close communication with Dietary Services and plans food activities at the beginning of each month. All special diets are discussed and decide if alternative food items are needed.
  • Participates in the MDS process and fills Activity portion that applies.
  • Follows infection control guidelines during interactions with the residents.
  • Participates in facility required in-service and provides and in-service annually regarding the purpose, function and need for resident activities.
  • Other duties as assigned.
  • Program, Lead and Engage residents with preferred activities.
  • Support households for success by understanding resident’s preferences and providing adequate schedule and supplies.
  • Be flexible and attentive to residents as preferences change.
  • Adapt programming to honor residents as preferences change.
  • Communicate and collaborate with care attendants, nurses and other applicable team members of resident vital information.

WORK ENVIRONMENT:

  • 75% Indoors, 25% Outdoors
  • Self-paced

PHYSICAL REQUIREMENTS:

  • Sitting: 2-3 hours per 8-hour work shift. May alternate frequently to standing or walking.
  • Standing: 1-2 hours per 8-hour work shift. May occasionally alternate to sitting and frequently alternate to walking.
  • Walking: 4 hours in an 8-hour work shift. May occasionally alternate to sitting and standing.
