What are the responsibilities and job description for the Adult Case Manager position at Spring River Mental Health and Wellness, Inc.?
Primary Function: To provide case management services to adult clients with a severe and persistent mental illness receiving outpatient mental health services through the community mental health center serving Cherokee County, KS.
General Duties and Responsibilities:
1. To provide quality, medically necessary case management services to adults with a severe and persisted mental illness.
2. Ability to meet productivity standards related to the provision of direct service expectations.
3. To assure that referrals for CPST or TCM services are acted upon and within 3 business days of referral. See definitions below.
4. To assure that clients will be assessed via the strengths/needs assessment tool.
5. To monitor and provide timely documentation in the electronic health record of all services provided in accordance with policy and SRMHW's documentation standards.
6. To attend workshops and complete training requirements as necessary for provision of CPST; TCM; and additional Rehabilitation Services, as determined by supervisor and/or state training requirements.
7. To assist with crisis intervention according to SRMHW’s policy.
8. To complete additional duties, as assigned.
Community Psychiatric Support and Treatment (CPST) definition: Goal directed supports and solution-focused interventions intended to achieve identified goals or objectives as set forth in the consumer’s individualized treatment plan. CPST is a face-to-face intervention with the client present.
Performance Requirements--Knowledge, Skills & Abilities: Educational background and working knowledge of working with individuals with a mental illness. Knowledge of strength based assessments. Knowledge of community resources and referral agencies. Skilled in establishing and maintaining effective working relationships with clients; staff; and the public. Ability to write reports and document client progress. Ability to organize and track program evaluation data. Ability to utilize computer software efficiently.
Job Qualifications: BA/BS degree in human service field strongly preferred. Will consider candidates that are equivalently qualified through a combination of years of experience and/or education. Possess good communication skills, ability to work cohesively as part of a team, ability to work with person with severe and persistent mental illness, and the ability to react effectively in a variety of human service situations. Prior experience should be in mental health or human services field. Meet specifications outlined by the Division of Healthcare Policy. Successful completion of background checks, child and adult abuse registry checks, pre-employment drug screening process, and motor vehicle record check for insurability as this position requires a valid drivers license and ability to transport clients in agency vehicles.
Starting hourly wage based on education, training and years of experience.
