Reports to: Director of Adult Day Services
Job Status: Full-time/ Non-exempt
Supervises: CNA and PCW

Position Summary: LPN with experience in working with clients with disabilities and seniors. Administers medical treatments to clients served under physician orders and records administration on prescribed medications and treatments.
Duties and Responsibilities:
- Performs the daily medical and nursing functions in accordance with current rules, regulations and guidelines that govern St. Ann Center for Intergenerational Care.
- Reviews client I.S.P. for appropriate care based on individual nursing needs and administer professional nursing services as needed and/or as ordered by the physician.
- Consults with the client's physician and caregiver, in close collaboration with the Director concerning treatment or when there is a change in client's condition.
- Performs administrative duties, which include completing medical forms, reports, evaluations and charting.
- Assures that St. Ann Center for Intergenerational Care personnel, clients and visitors follow established policies and procedures at all times, relative to nursing and medical needs.
- Contacts family/caregiver to refill prescribed medications, supplies, and equipment.
- Meets with clients and family members as necessary for any follow-up in-home care, in close collaboration with the Director.
- Assists with other client care as needed.
- Attends weekly staff meetings and quarterly all-staff meetings.
- Coordinates doctors orders with doctor's staff, home health agencies, and caregivers, and other health care facilities for continuity of care.
- Attends and participates in professional organizations inservices and meetings.
- Participates in weekly multidisciplinary staffing's.
- First responsible to all emergencies with appropriate follow up for all staff, child, adults, volunteers and guests at the Weinberg Building

Qualifications
Education/Training:
- Must possess a current, and valid LPN license in the state of Wisconsin
- Current CPR/BLS from AHA
