What are the responsibilities and job description for the Case Manager RN position at St. Barnabas?
The Case Manager is responsible for facilitating the patient’s hospitalization from pre-admission through discharge and uses available resources to provide the most appropriate and cost-effective care. The Case Manager interacts with physicians, nurses, social workers and other healthcare team members to expedite care and to ensure a seamless transition to the next level of care. The Case Manager identifies challenges early in the admission thorough interaction with the patient and family and effectively resolves them using clinical expertise, available data and evidence-based criteria. The Case Manager effectively researches readmissions with a goal of identifying the reasons for readmissions, addressing those reasons and eliminating them.
