What are the responsibilities and job description for the Hospice On Call LVN position at St. Bernardine Home Health and Hospice Care?
Our hospice agency is seeking experienced per diem Hospice LVNs in the High Desert area. We are looking for an On-call nurse to be available after hours for any issues that come up. Monday - Friday 5:30pm - 9am and all day Saturday Sundays.
We are looking for eager, caring, self-motivated, proactive and compassionate nurses who understand the special challenges that arise in hospice with patients and their family and who are able to provide care and comfort in a home type setting or skilled nursing facility.
Responsibilities:
- Collaborate with physicians and other professionals to plan care
- Make frequent visits to the patient to evaluate their condition
- Provide support and comfort according to individual patient’s needs
- Work with caregivers to plan and execute excellent patient care
- Help in administering medication
- Alleviate sudden symptoms of the disease’s end phase
- Notify a doctor for a patient’s condition when deemed necessary
- Assist patients and their families with paperwork. Maintain accurate records
- Help relatives cope with the upcoming loss and provide post-mortem support
Qualifications:
- Valid CA license issued by the Board of Registered Nursing or Board of Vocational Nursing & Psychiatric Technicians
- Preferred previous experience in nursing (at least 1 year)
- Familiarity with medical tools and equipment
- Ability to build rapport with patients
- Compassionate and caring
- Excellent written and verbal communication skills
- CPR-Current
- Driver’s license and car insurance
- Current history and physical with TB or Chest X-ray
Job Types: Part-time, PRN, Per diem
Pay: Up to $50.00 per hour
