What are the responsibilities and job description for the LPN SCMG position at St. Clair Hospital?
Under the general supervision of physician(s), Certified Registered Nurse Practitioner(s) (CRNP), or Physician Assistants (PA), assists physician(s), CRNP(s), and PA(s) in providing optimal healthcare to patients in the physician practice setting.
Minimum Qualifications
- Graduate of a recognized Practical Nursing program.
- Current licensure as a Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania OR current temporary licensure to practice as a Licensed Practical Nurse (LPN) in the Commonwealth of Pennsylvania, with successful completion of NLCLEX – LPN within 60 days of hire.
- Highly effective communication skills.
- Ability to navigate electronic medical records and web based applications.
- Valid driver’s license, motor vehicle insurance, and access to reliable transportation.
Preferred Qualifications
- Physician Practice nursing experience preferred but not required
- One year of prior experience working as an LPN
- One year of prior work experience involving direct patient care
Essential Functions

- Participates in assigned quality initiatives by following gaps closely and assisting the provider by placing orders prior to the visit in an effort to capture past due diagnostic testing and labs.
- Functions as a medical assistant by preparing the patient for the exam, documenting vital signs, updating medications, assist with exams, tests and special procedures as assigned.
- Participate in working the MA desktop in the EMR following Practice protocols and proper documentation and follow-up on all entries.
- Assist with ED follow-up calls when available and as assigned by the office manager.
- Assist with development of guidelines for prioritizing work activities, evaluating effectiveness, and modifying activities as necessary.
- Supports and upholds established policies, procedures, objectives, quality improvement, safety, environmental and infection control, and codes and requirements of accreditation and regulatory agencies.
