About the role
The RN-Hospice Liaison expands our visibility and referral base in the community and among health care agencies by; education, acting as a resource person and representing the Hospice Program during referral, intake, assessments, and discharge planning etc. Participates in outreach services to further the community's awareness of hospice services through in-services and meetings. Actively participates in providing the community information of both hospice care in the inpatient and home care setting.
Essential Job Functions
- Improves patient access to Hospice.
- Expedites referral/admission process.
- Meets regularly with UR/Discharge Planners, Medical Social Workers, Physicians, and Registered Nurses to evaluate potential Hospice candidates.
- Through the referral/intake process, meets with patents and families to discuss Hospice services.
- Markets/presents to the health care community/public that variety of services provided by St. Francis Hospice.
- Provides education to physicians, staff and families for specific subjects related to Hospice.
- Coordinates nursing service with other disciplines including social services, pastoral/spiritual care, and volunteer services when applicable.
- Ensures that the Interdisciplinary Plan of Care is kept current/updated of any changes and that the physician is updated on a regular basis as changes occur when appropriate.
- Teaches, supervises and counsels the patient and family members regarding the nursing care needs and other related problems of the patient when appropriate.
