9:00 am - 7:30 pm varied days
4- 10 hour shifts weekly
Under the general direction of the Admissions Director, the Admission Registered Nurse (RN) is responsible for determining the eligibility and preparation of patient/family need and the initial written plan of care. Position works closely with the Intake Coordinator and Clinical Service Department to ensure timely processing of all referrals. Position serves as a resource for questions/issues from the health care community and SFR staff. Position is full time and will include working weekends.
EDUCATION AND/OR TRAINING REQUIRED:
Requires an Associate degree in nursing. Bachelor’s or Certified Hospice and Palliative Care Nurse preferred.
CERTIFICATIONS/LICENSES REQUIRED:
Requires a valid current Florida Registered Nurse License and a valid Florida Driver’s License with reliable transportation.
EXPERIENCE/SPECIFIC SKILLS AND ABILITIES REQUIRED:
Three years hospice experience preferred. Previous sales experience preferred. Ability to work flexible hours. Must understand the dynamics of human behavior and ability to communicate with patients and families in a crisis. Ability to communicate effectively with referral sources, and colleagues. Excellent oral and written communication, clinical and administrative skills. Able to prioritize varied projects. Dependable, flexible and tactful.
PRIMARY RESPONSIBILITIES:
- Assists the Intake Coordinator with the coordination of referrals.
- Meets the needs of the newly referred patient / families, completing the admissions process within 24 hours or sooner.
- Gives a comprehensive explanation of hospice services, philosophies, and benefits addressing all components of care.
- Obtains signatures on all necessary consents ensuring that patient / families are informed of their rights and responsibilities.
- Performs a physical / environmental assessment, in addition to a psychosocial and spiritual assessment.
- Provides primary clinical care and teaching, as needed, to stabilize the patient and facilitate the admissions process.
- Documents assessments and establishes the initial plan of care and plan of treatment after consultation with another team member.
- Is available and flexible with scheduling to meet the admission needs of the patient / families.
- Is an active, enthusiastic member of the admissions team providing support for colleagues and creative ideas for improvement of the admissions process.
- Maintains strict confidentiality and respect for patient / family privacy and possessions.
- Provides emotional support to patient / family and other caregivers.
- Increases knowledge of palliative care through active participation in educational opportunities.
- Assists patient / family with understanding requirements during implementation of a disaster plan.
- Participates in orientation and education of SFR personnel.
- Participates in performance improvement and quality control activities.
