Social Services Director

Social Worker needed for 128 bed long term care facility. Facility is privately owned and has a great work environment which helps staff members unify and grow together while achieving our goals of caring for all of our residents. Our facility is dedicated to serving its residents and is looking for someone as equality motived as we are. We are located a short drive from Zachary, Baton Rouge and New Roads. Some duties that are required for this position need to be preformed by a certified/registered social service director in the state of LA. These job duties include but are not limited to:

-Case management/Discharge planning

-Conduct assessments according to CMS requirements

-Care plan implementation and evaluation

-End of life and Advanced care planning

-Assist residents with completion of benefit enrollments

-Assist residents and families with general services and admission into the facility

-Complete PASSR and 142 continued stay requests

Must be computer proficient, team oriented, caring, punctual, and innovative. Skills involving time management and organization will assist in completing all tasks this position entails.

Job Type: Full-time

Pay: $40,000.00 - $47,000.00 per year

Schedule:

• 8 hour shift
• Monday to Friday

Education:

• Bachelor's (Preferred)

License/Certification:
