Pay: $39.86 - $49.37 per hour, depending on years of experience.

Job Summary:
The Social Worker performs a variety of services under the general supervision of the Director/Manager, with the main focus of linking patient to the community and with community resources and providing psychosocial to vulnerable patients/families to meet their needs. Home visits/Community visits required whenever warranted.
Requirements/Skills:
- Masters of Social Work
- A valid N.Y.S LMSW or LCSW required.
- Spanish/English bilingual preferred
- 1-2 years of experience in outpatient health working with adults and children
- Must be able to problem solve and think critically.
- Must be computer literate, familiar with Microsoft Word and Excel.
- Must be highly organized, work with the interdisciplinary team.
Salary : $40 - $49
