- Associate Degree plus 2 years of experience in a social service field
- Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field
- Valid California Driver's License required
- Has responsibility for processing referrals
- Ensures all required forms, consents, legal documentation, etc. are in place, and that related activities have occurred in a timely manner.
- Interacts with clients, families, referral sources, outside agencies, and CS staff in a professional, efficient, and quality focused manner
- Provides services in the community, and in client’s homes. Uses personal vehicle to drive within the community to provide services and to transport client and/or client’s family members in personal vehicle.
- Documents all services provided in a clear and concise manner within the Electronic Medical Record within the timelines and framework established by Central Star.
- Outpatient community-based programs
- Serves clients (ages 0-18)
- Connect people with community resources, including vocational resources for young adults
- Group therapy and family Counseling
- Medication management and support service
