REHABILITATION COUNSELOR - (FULL TIME)

DEPARTMENT:       Single Room Occupancy (SRO) Program - Jamestown, NY                                          

SCHEDULE:            Tuesday, 9:00 am to 5:00 pm

                                   Wednesday, 9:00 am to 5:00 pm

                                   Thursday, 9:00 am to 5:00 pm

                                   Friday, 9:00 am to 5:00 pm

                                   Saturday, 9:00 am to 5:00 pm 

SALARY:                Minimum $19.00/hr.

AVAILABLE:           ASAP

JOB FUNCTION:

Provide case management, rehabilitative, and therapeutic services to residents. Teach, counsel, and monitor residents’ activities of daily living in a community residence or apartment setting, developing skills, abilities, and experience that will facilitate a greater opportunity for the resident to live in an independent environment.

MINIMUM QUALIFICATIONS:

Education, Experience, and Knowledge

• Minimum education and experience: high school diploma/GED and customer service experience. Healthcare or behavioral health experience is a plus.

• Preferred Education and Experience: Bachelor’s degree in a human services-related field or an associate degree in a human service-related field and three years experience in human services or a high school diploma/GED and five years experience in human service. 

• Must possess a working knowledge of current issues and concerns of people with mental illness, rehabilitative training methods, and community resources.

OTHER QUALIFICATIONS:

• Have a home/cell telephone.

• Be able to travel to and among various multi-level facilities with stairs only.

• Have a valid driver’s license and a safe driving record.

• May be required to use personal vehicle for programmatic purposes.

• The NYS Office of Mental Health, NYS OMIG, and other regulators require criminal background checks and other pre-employment checks.
