Social Worker-Home Hospice

Stella Maris, located in Timonium Maryland, is looking for a Home Hospice Social Worker to join their growing team. This is an exciting opportunity to work alongside dedicated hospice clinicians to meet the needs of clients in the community we serve.

Under the direction of the Home Hospice Director, the main function of this position is to provide social work services to our Home Hospice patients and their families.

**This position requires daily travel in an assigned area**

This position provides direct social work services to home hospice patients and families.  Services include admission, psychosocial assessment, plan of care, linkage with community resources, counseling, crisis intervention, and discharge planning.  Assumes responsibility for required documentation in compliance with State and federal regulations.

• Responsible for required documentation in compliance with State and federal regulations.
• Assesses bereavement issues of families and makes timely and appropriate referrals to Center for Grief and Loss.
• Facilitates effective communication between patient/family and home care team members.
• Provides appropriate information and referral services to patients and families/caregivers.
• Assumes responsibility for discharge planning as needed

#INDHP

• Master’s Degree in Social Work 
• Social Work License in the State of Maryland 
• Experience in hospice or medical setting preferred
