JOB SUMMARY:
Responsible for development and implementation of activities intended to educate, market, and inform professionals and communities of SMC Rehabilitation Program and services. The Internal Liaison serves as admission screener for rehabilitation candidates within Stillwater Medical Center and the Stillwater Community, with completion of Admission Screens to determine patient candidacy in accordance with policy and CMS guidelines. The External Liaison serves as admission screener for rehabilitation candidates beyond Stillwater Medical Center and the Stillwater Community, with completion of Admission Screens to determine patient candidacy in accordance with policy and CMS guidelines. Contributes to maintaining a successful Program census on the Rehab Unit at all times. Liaison works closely with the Manager of Admission Coordination and Compliance and Medical Director to ensure success and cost-effectiveness of the Program.
QUALIFICATIONS:

Graduate of a program in Physical Therapy, Occupational Therapy, Speech-Language Pathology, Nursing, or Social Work approved by a nationally recognized accredited body.

Applicable and current license to practice is required.

Experience working with adult and geriatric patient populations.

External Liaison Only: Having and maintaining a valid Oklahoma Driver’s License and satisfactory driving record is a condition of employment.
PHYSICAL REQUIREMENTS:

Demonstrates adequate vision, or correctable with glasses/contacts, to prepare and process information, supervise practice of employees, and monitor the environment.

Must be able to talk and hear to converse with others, in person and over the telephone.

Must have adequate perception of sounds or adequate hearing with corrections.

Demonstrates adequate stamina to deal with stressful situations and complete necessary work schedule.
