Stonecreek Health and Rehabilitation is seeking a Social Services Director!
The Social Services Director must have a BSW or MSW in Social Work or a related field.
While we prefer a candidate to have experience in long term care, a passion for serving the elderly matters most.
This position assists with assessment and documentation associated with care planning, behavior management and discharge planning. The Social Services Director works closely with residents, loved ones, and community resources in order promote quality of life and person centered care.
