Position Summary

The Summit Community Care Clinic (SCCC) Registered Nurse will promote, maintain, and improve individual and community health by supporting patients in adopting healthy behaviors. The Registered Nurse will provide safe, effective, and quality primary, preventive, and evidence-based health care to patients in a culturally competent manner, focusing on health promotion and disease prevention. The Registered Nurse is responsible for assisting medical providers and clinic assistants in the efficient and orderly delivery of medical services as well as performing related duties as assigned by the chief nursing officer. The Registered Nurse acts as a resource to the clinic assistants, front desk staff, health educators, and providers.

The Registered Nurse must demonstrate regard for the rights of all patients and provide respectful and considerate care focused on the patient's individual needs. The Registered Nurse will provide a safe and confidential environment for patients, families, and personnel.

Essential Functions

  • Deliver quality service creating solutions that add value to a patient’s health.
  • Implements and coordinates all aspects of SCCC’s Immunization program including compliance with Colorado Department of Public Health and Environment’s requirements.
  • Administer, receive, store, and organize vaccines as well as monitor temperatures.
  • Commit to continuing education regarding immunizations.
  • Provides clinical support to clinic assistants, health educators, front desk staff, and providers.
  • Accurately document patient interactions/information in the patient’s electronic health records.
  • Complete paperwork regarding vital signs, allergies, etc.
  • Accurately chart lab results, and send reminder notices, and other notes in a timely manner.
  • Focus on quality customer service for all patients, coworkers, volunteers, and partners outside SCCC.
  • Participate in the Clinic’s QI/QA Program as directed by the supervisor. This may include collection, reporting, and/or review of quality data and participation in performance improvement projects.
  • Provides clinical and non-clinical in-services for SCCC staff.
  • Support clinic assistants with checking and documenting patient phone messages and prescription refill phone line, processing prescription refills according to protocol.
  • Assist medical providers with patient procedures and testing as needed.
  • Assist the Chief Nursing Officer and Chief Medical Officer with projects as assigned.
  • Maintain compliance with Occupational Safety and Health Administration regulations. Responsible for maintaining knowledge of these regulations for the application of guidelines in the workplace.
  • Maintain all aspects of patient confidentiality according to HIPAA regulations.
  • Refers to and consults with other health care professionals as needed.
  • Educates patients and family members about health care issues.
  • Attends staff meetings.
  • Actively participates as a member of the integrated health care team, working in conjunction with the Care Team, and Dental and Behavioral Health departments to ensure seamless, multidisciplinary patient care.

Additional Tasks/Responsibilities

  • All jobs at the Care Clinic are impacted by various grants that are necessary for our sustainability. Serving in this role has direct responsibilities to those grants and is vital to the success and continuation of those grants.
  • Performs other related job duties as assigned by the Medical Director.
  • Complete all required training as designated by HR and the position supervisor.
  • Adhere to all policies within the organization and those outlined within the SCCC Employee Handbook.
  • Comply with all SCCC Vaccine Mandates.
  • Willingness to travel and work at other SCCC locations as determined by the supervisor.
  • Comply with all applicable Credentialing and Privileging requirements timely.
  • Embrace the SCCC Mission and Vision and agree to work within the SCCC Guiding Principles and the principles set by the position supervisor.

Education/Experience

Must Haves to be successful:

  • Bachelor’s degree in nursing or advanced Degree in Nursing.
  • Current Licensure as a Registered Nurse from the State of Colorado.
  • Current CPR card.
  • At least two years experience in a clinical setting.
  • Ability to read, write, and speak the English language.
  • Ability to interpret a variety of instructions and deal with multiple linguistic and cultural variables.
  • Ability to define problems, collect data, establish facts, and draw valid conclusions.
  • Ability to exercise independent judgment.
  • Ability to reason objectively.
  • Ability to assess, project, and plan for patients’ needs.
  • Ability to interpret state/federal/agency regulations.
  • Ability to document concisely, accurately, and in a timely manner.
  • Ability to handle a variety of duties that may be interrupted and/or changed by immediate circumstances.
  • Ability to relate cooperatively and constructively with patients, co-workers, administration, health care providers, community agencies, referral sources, regulators, and other health team members.
  • Ability to enlist the cooperation of others.
  • Ability to function well in a high-stress environment.
  • Competent knowledge of Microsoft (Word, & Excel), and database software.

Nice To Haves

  • Previous non-profit and/or volunteer experience preferred.
  • Ability to speak the Spanish language (bilingual) preferred.
  • Highly organized.

Supervisory Responsibilities: None

The environment is in a healthcare setting which can be fast-paced and involves working directly with patients and healthcare professionals. The workspace is primarily a clinical setting, but the individual may also work in a clerical setting. Exposure to infections and contagious diseases may occur. Many of our patients with chronic diseases have a minimal level of health literacy and a significant number of barriers to accessing health care.
