What are the responsibilities and job description for the Social Worker position at Sunlight Adult Day Health Care Center?
Adult Day Health Care Center (ADHC) is seeking a Social Worker to join our dynamic team! The Social Worker will assume responsibility for a caseload of approximately 40 adult participants, most of whom have one or more medical diagnoses. The Social Worker will provide a range of services including case management, community resource referrals, counseling to elderly and other physically/mentally impaired adults in a day program setting.
Requirements:
-Bachelor’s degree in Social Work from an accredited school with 2 years of experience providing social services in one or more of the fields of aging, health, or long-term care services OR
-Master’s degree in Social Work from an accredited school of social work OR
-Master’s degree in Psychology, Gerontology, or Counseling from an accredited school and 1 year of experience providing social services in one or more of the fields of aging, health, or long-term care services
-Valid Driver's License and proof of insurance
Responsibilities:
-Conduct biopsychosocial assessments (includes home assessment)
-Case management (schedule appointments, coordinate transportation, assist with access to community resources, etc.)
-Develop and implement person-centered treatment plans, with the ability to determine appropriate milestones to gauge progress
-Provide individual and family counseling
-Attend multidisciplnary team meetings, communicate updates to relevant parties
-Make referrals to other healthcare providers as needed
-Complete all documentation and reports in a timely and accurate manner
-Community outings
-Group facilitation
-Marketing and outreach
-Intakes and discharges
Qualifications:
-Must be energetic, self-motivated, organized, and efficient with completing tasks and meeting deadlines
-Must be comfortable working in a culturally-diverse setting, and utilizing on-site translation services when necessary.
-Must have excellent oral and written communication skills, with basic proficiency in Microsoft Office (additional computer program training will be provided
-Must be willing to assume additional job duties as assigned; center staff operates as a team, and all staff fill in with additional tasks when necessary (assisting with activities, meal service, answering telephones, filing, etc)
-Knowledge of community resources in Modesto and surrounding areas a plus
-Bilingual preferred, but not required (Spanish, Assyrian)
