What are the responsibilities and job description for the MDS Coordinator | RN position at Sunnyview Nursing Home?
Responsibilities include:
- The coordination and completion MDS assessments and care plans for assigned residents.
- The maintenance of resident care plans.
- Participation in any facility meetings as warranted.
- Monitor and Audit clinical documentation to ensure accuracy.
Qualifications:
- Licensed in the state of Rhode Island as an RN with long term care experience.
- Knowledge of MDS and careplan process.
- Minimum of one year of MDS experience in a long-term care facility.
- Should possess the ability to be accurate, concise, detail-oriented and able to work cooperatively as a member of the inter-disiplinary team.
- Should possess the ability to organize, prioritize, work in a time-sensitive environment, and communicate within the organization.
- Should possess excellent assessment and documentation skills.
