Job Title: Intake Clinician
Department: Clinical
Reports To: Clinical Director
POSITION SUMMARY:
Serve as first point of contact to warmly and genuinely welcome new patients and accompanying guests. Discerningly assess patients to determine appropriate level of care, including performing Biopsychosocial Evaluations and risk assessments.
ESSENTIAL DUTIES AND RESPONSIBILITIES: This will be a dayshift position with a flexible schedule. Compensation will depend on qualifications. As a intake clinician you will complete initial paperwork, release of information forms, and other vital intake procedures. Organize and leads new patient orientations to ensure all new patients are oriented to facility policies and procedures.
- Meet with potential patients, address questions about programming and provide a facility tour.
- Competent in diagnosing and treating clients with substance abuse disorders.
- As staffing patterns require, fill in for Therapists by leading therapy groups and individual therapy sessions.
- Monitor notifications and respond promptly.
- Work cooperatively with all levels of employees, management, and external agencies.
- Conduct initial clinical evaluations.
- Provide individual, family, and group services periodically as needed.
- Communicate effectively with the Utilization Management staff.
- Collaborate and communicate with all parties involved in working towards client’s identified goals.
- Reporting and Documentation
- Keep timely, thorough, and accurate progress notes. Document next steps, and update treatment plans accordingly.
- Timely completion and maintenance of case and agency paperwork according to Sunrise standards and as required by external agencies (Joint Commission, State Regulations) as well as internal unit and organizational reports and documentation.
- Participate in appropriate team and/or administrative meetings, assigned committees, in services, and other continuing education/training.
- Other related duties as assigned.

Education and Training:
Master’s degree in Social Work, Psychology, Counseling or other mental health related field or a Doctoral degree in Psychology from a college/university accredited by the US Department of Education required.
