Job Description
The ideal candidate will possess excellent relationship-building skills, a strong understanding of providing nursing services to residents while promoting the highest level of well-being as directed by the Health Services Director and Regional Staff.
Job Responsibilities:
- Administer medication and treatments according to health care providers' orders and state regulations.
- Assist resident care coordinator in creating individualized service plans based on resident needs.
- Dementia/Memory Care programming
Report changes of condition and work directly with the Director of Nursing to ensure that regulations and internal standards are met.
language.- Monitor resident responses to treatment plans and identify adverse medication interactions, alerting the resident care coordinator of any issues
- Update resident records with information regarding nursing care, treatment plans and observations

Qualifications
Minimum job requirements:
- Current Licensed Nurse (LPN or LVN) in the state of practice.
- 1-5 years of nursing experience, preferably in an assisted living environment.
- Proficiency with computers and Microsoft Office program.
- Flexible schedule, including availability to work evenings, weekends and holidays as needed.
- Superior written and verbal communications skills, coupled with highly developed organizational skills, problem-solving and attention to detail.
- Able to work well independently and as part of the team, while working to ensure team goals are met or exceeded.
- Must enjoy working with the senior population.
