Job Summary:

The Department of Social Work at SUNY Downstate Health Sciences University is seeking a full-time Social Worker 2.

The Social Worker is an integral part of the hospital multidisciplinary team. The Social Worker works with the patients, their families and caregivers to assess their psychosocial and mental health needs. The Social Worker is responsible for the coordination and delivery of care services to patients and their families. They educate patients, family and staff on diagnosis, treatment programs and available resources.

The Social Worker also conduct psychosocial, emotional and socioeconomic assessment of children with, life threatening diseases and collaborate on care plans with evaluation of patient outcomes. They work collaboratively with other members of the patients' care team to assure high quality of care. They provide crisis intervention, emotional support, resource information, and legal reporting when needed, long-term Care planning, etc. The Social worker's goals are to:

Provide overall coordination for mental health and social services for patients of SUNY Downstate Primary Care Ambulatory Care Department.

Assess the psychosocial needs of patients and refer them to appropriate social services and behavioral health services, internal or external as needed.

Create individual care plans for all patients that are actively engaged in social worker services.

Engage family members/caregivers to discuss patient status and provide supportive counseling and guidance where indicated.

Assist patients with obtaining durable medical equipment. medications, medical supplies and all other services that will Improve patient borne environment and self-management abilities.

Connect and reconnect patients to long term care services such as managed Long-term care Plan (ML-TC); FIDA; MAP, HARP program.

Collaborate in a team-based care model with all members of the patient care team including PCPs, nursing, behavioral health and support staff, including:

Attending and participating in daily clinic huddles.

Receiving warm, handoffs from PCPs, nurses and other clinicians.

Providing feedback to PCPs on a regular basis in regards to the status of the patient and all Interventions provided.

Participate In at least weekly caseload review with supervisor.

Document in EMR all Interventions and interactions with patients, caregivers, hospital divisions and external agencies, including follow up and disposition of cases.

Maintain a tracking system for active and Inactive patients to reflect status and disposition.

Communicate with all individuals and groups required to ensure appropriate referrals are made and loops are closed, including but not limited to, patients, patients' families and caregivers, SUNY Downstate divisions, care coordinators and other relevant staff at external agencies, and Insurance companies.

Interact and communicate with Inpatient discharge planning, staff, social work and care management to manage care transitions for high-risk patients.

Utilize online available community resource directories to access most effective and efficient services for patients.

Required Qualifications:

1 year of social work experience. Hospital experience, CCM preferred.

Knowledge of Federal, State and TJC regulations.

Knowledge of Medicare, Medicaid and Managed Care Standards, requirements, benefits and benefits administration.

Knowledge of state and federal programs that provide financial support to underserved population.

Knowledge of community resources.

Proficiency in use of PC, Microsoft Office Suite and clinical Applications.

Ability to work with diverse patient population with knowledge and sensitivity to cultural and spiritual influences, impacting patient care social work assessment and counseling skills, grief and bereavement counseling, alcohol and drug intervention, and support for victims of abuse, neglect and violence.

Possess a positive attitude, motivation and good interpersonal skills excellent verbal and written communication skills. Good problem resolution skills. Ability to work in a multidisciplinary-team approach. Ability to work independently with minimal intervention.
