Job Summary:
In this position, the social worker provides comprehensive psycho-social assessment, management and support to at risk patients and families at the Upstate Pediatric and Adolescent Center. Duties will include consultation through assessment, counseling, intervention, planning and referrals to meet the psycho-social needs of patients and families. In addition the social worker will actively manage patients through collaboration, coordination, monitoring, evaluating options and identifying required services to meet individual's health needs. The social worker rounds on their assigned patients with the multidisciplinary team. Additionally, the social worker will participate in quality meetings with on their assigned unit or clinic. Candidate must possess communication skills, have knowledge of Social Determinates of Health and available community resources, document all interventions; participate in departmental meetings/in-services, education, and quality improvement activities; and perform other relevant duties as assigned to include coverage for other social workers and back-up responsibilities.
Minimum Qualifications:
M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receiving limited permit.

Monday-Friday 8:30am - 5pm, Occasional on-call/holiday coverage on rotation based on operational need
